Farmhouse is not separate from Milestones. It is Milestones care held intimately — for guests who need more privacy, more discretion, and more room to be closely supported.

Located in a private residence on our Tennessee campus, Farmhouse is designed for up to five guests at a time. The work is aimed at the source of wounds, not only the symptoms.

Care is coordinated across every dimension — structured clinical work, farm-to-table meals, restorative somatic supports — to steady the nervous system and create conditions in which deeper work can happen.

PHILOSOPHY

The goal is not simply relief. It is to help each guest understand their wounds, understand how they have shaped their patterns, and know what kind of support will be needed for healing to hold beyond the stay.

Rooted in Milestones’ deep clinical experience in trauma treatment, Farmhouse is built on the understanding that trauma affects the whole person. Care is coordinated across the full experience so emotional, relational, physical, and psychological healing can be supported together rather than in sequence.

Individual Therapy

Three therapy sessions each week — dedicated one-to-one time for the work that needs closer attention.

Whole-Person Evaluation

Functional medicine and psychological diagnostic evaluations give the care team a more complete picture: emotional, relational, physical, and psychological.

Relational Intensives

A two-day family or couples intensives are included every 30 days, allowing communication, repair, and integration begin to happen while care is still active.

Group Therapy

Trauma happens in community. Healing also happens in community. Group offers a setting to be seen, challenged, and connected – at a scale where no one disappears.

Equine Therapy

Individual and group equine work are both built in, offering another way to explore trust, boundaries, and connection.

Around the table, nourishment becomes part of the treatment — a way to slow down, be cared for, and return to the body.

Farm-to-table meals are prepared on premise by our chef, with ingredients sourced from the property when possible and designed to support the body through demanding clinical work.

Care extends into the rhythm of the day. Movement, nourishment, rest, body-based support, and restorative spaces create conditions in which the nervous system can settle, and deeper work becomes possible.

Weekly therapeutic massage, daily vibroacoustic therapy, sauna, cold plunge, support rest, recovery, and nervous system regulation.

A private gym, walking paths, and open air create space for movement, reflection, and time outside of a room.

Quiet spaces throughout the house and property allow guests to pause, reflect, and integrate the work of treatment.

The Farmhouse mark brings together two symbols rooted in the life of the land: the horseshoe and the maypop. The horseshoe grounds us in strength, presence, and the quiet wisdom of the herd. The maypop – Tennessee’s native passionflower – carries a story of resilience, complexity, and return.

Together, they hold the heart of Farmhouse: a deeply personal experience, shaped so that the real work has room to take root.

What is a typical day like at Farmhouse?

There is structure, and there is room inside it. A day is built around your three weekly individual sessions and group work, with the rest shaped to what your treatment plan calls for. A day might include a morning lecture, group therapy or individual work, meditation, equine therapy, vibroacoustic therapy or massage, time in the gym or on the walking trails, meals prepared by our chef, 12 Step Meetings, and unhurried time to rest and integrate. Because the house holds five guests, the schedule can flex in a way a larger program cannot.

Each day is specialized for each client. There is no “cookie-cutter” day at Farmhouse because we believe your care should be as unique as you are.

What is the campus like?

Farmhouse sits within our campus in 250-acres of among the rolling hills of Tennessee, just outside Nashville. The property is designed for recalibration and reconnection – pasture, walking paths, the farm and garden, and the horses.  The house itself is set apart from the main campus, which provides restorative privacy. 

What are the accommodations?

Each client has a private room with a private bathroom, high-quality linens, and toiletries. Every space in the house was designed for rest – the same care you would expect of a good home, applied to a place where healing is happening.

What food is served during a program?

Farm-to-table meals are prepared by a private chef to nourish the body throughout the stay.

Can you accommodate my specific dietary needs or restrictions?

We provide nutritious meals for various dietary needs. Each meal includes gluten-free, vegan, vegetarian, dairy-free, and soy-free, as needed. Our team is committed to doing everything we can to ensure you have food that nourishes and comforts you in the midst of your emotional work. 

Do you accommodate Kosher?

Yes – including a dedicated kosher refrigerator, microwave, and coffee setup, dedicated preparation equipment, kosher meal options at lunch and dinner, a stocked pantry, and candles and a place to observe Shabbat. Our team will walk through the details with you directly.
